It is available only for low income patients who have to show that they are already eligible for both Medicaid and Medicare. These programs do not operate everywhere, but you can check and see if one is available near you. 

Veteran's Affairs (VA)

Veteran’s benefits can cover two different groups of people: the veterans themselves, as well as their surviving spouses. The Department of Veteran’s Affairs can cover a variety of different health benefits too. For in-home care, this can include a monthly cash stipend that will help defray the cost. Of course, this is only available for people who have fought in the military or who married a veteran.
